Pace Driver

Lafayette Franciscan Healthy Living Clinic 1116 N 16th St Lafayette, Indiana 47904 The PACE Driver is responsible for providing safe transportation to participants in a timely, competent manner and communicating information relevant to the plan of care with the interdisciplinary team. This position reports directly to the PACE Transport Supervisor. PACE drivers are required to demonstrate the knowledge and skills necessary to provide service to frail elderly participants according to assigned responsibilities and PACE Program standards. The PACE Driver is critical for setting the tone and experience expected by each participant on trips to the center and external appointments. The PACE Driver has direct interaction with caregivers and contracted partners while conducting business on behalf of the Franciscan Senior Health and Wellness (FSHW) Program of All-Inclusive Care for the Elderly (PACE) program. The PACE program's vision is to provide individualized and joyful care through exemplary teamwork, serving as many seniors as possible with the best quality-of-life in their communities. PACE offers seniors and their families the care, nutrition, rehabilitation, transportation, and supportive services they need to remain healthy so that they can live in their own home. What You Can Expect

Practice Hours: Monday Friday, 8:00 a.m. 5:00 p.m. No Weekends, Evenings, or Holidays Safely carries out approved transfers of participants to and from vehicles, at appointments, in participant homes, and in the community setting. Assists passengers with physical or cognitive impairments on and off vehicles in a safe and courteous manner. Uses medical equipment prescribed by members of the IDT to physically assist participant transfers and mobility, not limited to power wheelchairs, manual wheelchairs, walking aids, oxygen and others. Secures all medical equipment safely in vehicles during transportation. Supervises passengers during transportation. Escorts participants to and from home, PACE center, and authorized external/specialty appointments. Attends appointments with participants for safety as deemed necessary by the IDT. Responsible for vehicle and participants during transport, providing equipment and service in a manner that promotes participant satisfaction and retention. Follows established schedules and adjusts to meet unanticipated changes. Determines the safest and most efficient routes while considering the needs of participants and recommendations from the PACE IDT. Implements roadside safety precautions and manages safety of participants. Coordinates response and reports accidents to the PACE Transportation Supervisor. Follows established procedures to respond to non-medical emergencies. Activates emergency response in medical emergencies on the road. May serve as The PACE Driver designated representative of the PACE IDT to meet CMS regulations. As a PACE IDT member, the PACE driver takes part in shared decision making, authorization, and care responsibilities to provide required services. Reports any participant concerns, service requests, and safety observations to appropriate IDT member(s). Escalates important information to proper staff of Franciscan Alliance PACE daily regarding any changes, progress, or unusual events concerning any participant. Inspects vehicles and maintains them in a clean and safe working condition and maintain daily inspection log. Notifies Transportation Supervisor of maintenance or repair needs. Ensures that vehicles have enough fuel for the next shift. Delivers medications and other supplies to participants and documents delivery in EMR. Documents all transport encounters and deliveries in Electronic Medical Record (EMR). Responsible for documenting service requests and informing appropriate IDT member(s) of service requests as required by PACE regulations. Follows proper infection control procedures and maintain supply of emergency clean up kits. Qualifications
