Purpose
The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. Our work is guided by the principles of stable housing, a culturally competent and anti-racist lens, and respect for each individual’s journey.
Position Overview
The Social Worker is responsible for providing clinical intervention within the program, delivering culturally competent and sensitive services. As part of a therapeutic team, the social worker collaborates with the division’s medical, psychiatric, and psychology staff to ensure appropriate treatment interventions for residents.
Key Responsibilities
- Carry a caseload of residents and provide services within program regulations and the agency mission.
- Complete intake assessments, overall client assessments, and risk assessments.
- Evaluate clients’ functioning and support them in meeting individual goals.
- Perform person-centered, collaborative service planning; ensure service plans are completed and signed according to regulatory guidelines.
- Interpret and explain agency policies and procedures to residents.
- Develop safety plans; provide individual and family counseling.
- Make referrals and coordinate services (medical, mental health, substance abuse prevention and treatment).
- Advocate on behalf of residents with other service providers.
- Link residents to community-based programs and collaborate with other service providers.
- May run groups under supervision.
- Participate in audit preparation.
- Other duties as assigned.
Team Process
- Participate in weekly case conferences, intake, assessment, discharge, and follow-up conferences.
- Attend regular staff meetings and trainings; travel to offsite meetings as needed.
- Participate in planning and execution of special and holiday events.
Documentation
- File incident reports as appropriate.
- Document all resident interactions timely, including progress notes.
- Facilitate monthly roommate meetings when appropriate.
Competencies
- Excellent engagement and assessment skills
- Strong verbal and written communication skills
- Attention to detail; ability to work independently and as part of a team
Educational / Training Required
- Master’s degree in Social Work; LMSW or LCSW certification preferred
- Minimum of 1 year experience in Human Services preferred
- Willingness to work with individuals with experiences of mental illness and substance use
Computer Skills
- Familiarity with Windows environment; ability to learn new systems
Visual and Manual Dexterity
Ability to read data and documents (including spreadsheets and EHRs) and input data into relevant systems.
Work Environment / Physical Effort
Travel in the community may be required.
