Our local Chicago home health agency has helped 1000's of patients over the last 22 years and is looking for an experienced Director of Nursing to join our small team of dedicated individuals. We take a lot of pride in treating both our patients and employees with respect and care. This pivotal role includes managing the clinical aspects of the agency and other agency functions to operate effectively and efficiently on a day-to-day basis. The main priority is to ensure that the patients receive the best plan of care that will lead to the best patient outcome. Other high priorities include making sure nurses are fully supported, clinical documentation meets/exceeds Medicare Guidelines/Procedures and keeping up with any changes in the home health industry that may impact the agency. Salary - $95,000 - $106,000/yr Role Description Responsibilities
Ensure Medicare guidelines and CoPs are being met including clinical documentation. Must be available in person or telephonically during business hours to agency, staff, patients, and families as needed. Develop and maintain a Quality Improvement program to ensure quality patient care through clinical record reviews, implementation of corrective action where appropriate and program evaluation. Coordinates, directs, and manages the clinical operations of the agency. Participate in the interviewing, hiring and evaluation of qualified nursing personnel. Direct and coordinate adequate orientation to the agency and services provided by the agency for all newly hired agency personnel. Direct, plan and implement ongoing in-service programs and the annual competency evaluation program for all agency nursing staff. Manages and interprets agency policies and procedures to clinical staff. Ensure the provision of home health care to patients and families in compliance with physician orders and the established treatment plan. Provide for and actively participate in the annual agency program evaluation. Provide for and maintain a system for the acceptance of patients, obtaining physician orders, and the assignment of patient care to appropriately qualified clinical staff. Provide for and maintain an adequate system for staff communication which ensures coordinated implementation of physician orders, the patient’s treatment plan and any changes to the patient’s care. Ensure that all discipline specific professional standards are met in the provision of patient care by clinical review of the assessment, treatment planning and visit note documentation. Provide supervision, feedback and training when needed. Coordinate clinical review process for disciplines other than nursing with the discipline specific supervisory person. Review MD notes, Hospital visit notes, and referrals to make sure they are acceptable. Order labs, and nursing supplies. Assumes additional responsibilities and works on assigned special projects as determined by the agency Administrator. Must ensure office staff collect nursing and therapy paperwork on time so that billing is done.
