JAMHI Health & Wellness, Inc. is seeking a dedicated and client-focused Care Navigator to ensure smooth access to care within our integrated clinic.

Reporting to the Director of Practice Management, this role serves as the first point of connection for new and existing clients by coordinating appointments, tracking referrals, and ensuring services are aligned to meet individual needs. You'll provide in-person support, assist clients in understanding Medicaid and Medicare eligibility and benefits, and address basic needs that may stand in the way of receiving care.
This position is ideal for someone who thrives in a collaborative, fast-moving environment and is committed to making sure every client has a clear path to the care they need.
This is a full-time, permanent position with working hours of Monday through Friday from 7:30 a.m. until 4:00 p.m. Wage placement is based on relevant work experience and education.
Minimum Qualifications:
Associate's degree in human services, public health, or a related field
Similar experience in healthcare, behavioral health, or social services can substitute for education on a year for year basis.
AND
One year of experience in care coordination, case management, or patient navigation using an EHR One year of experience navigating Medicaid, Medicare, and basic public benefit programs. (Can be the same position as above) Preferred Qualifications:
Bachelor's degree in human services, public health, or a related field Familiarity with Alaska's Medicaid and Medicare programs Bilingual or multilingual ability
