Health Information Management Analyst - Commack Position Summary At Stony Brook Medicine, the

Health Information Management Analyst

plays a key role in maintaining the accuracy, completeness, and quality of patient medical records. This position is responsible for reconciling all discharges, assembling and prepping records for scanning, and ensuring that required documentation such as discharge summaries, operative reports, and history and physicals (H&Ps) are present, timely, and properly filed. The Data Quality Analyst reviews and analyzes medical records to identify documentation deficiencies, track compliance, and ensure the integrity of the medical record. Additional responsibilities include verifying and correcting patient demographic information, reclassifying misfiled documents, tracking unsigned provider orders, tracking incomplete ED physician documentation, and scanning/indexing relevant prior H&Ps as needed. This role supports overall HIM operations and contributes to ensuring that medical records meet organizational, regulatory, and quality standards. Duties of a Health Information Management Analyst

may include the following but are not limited to: Review medical records to ensure H&Ps are present and completed within 30 days

Scan and index prior H&Ps if missing and relevant to the current visit

Review documents for incorrect note types and correct any misfiled entries

Track unsigned provider orders, including verbal and refused orders

Verify and correct inaccurate patient information within the medical record

Track ED physician note completion

Analyze records after discharge for completeness, as needed

Enter deficiencies into the computerized chart management system, as needed

Assemble and prep charts for imaging, as needed

Remove received charts from the reconciliation queue, as needed

Other duties within this and related areas of HIM as required

Required Qualifications: Associates degree in HIT or related field or enrolled in an HIM program with an expected completion date within four years or enrolled in a HIT program with an expected completion date within two years.

Preferred Qualifications: Bachelors degree in HIM, Informatics or a related field

2 or more years of related healthcare experience

HIM experience

Strong demonstrated knowledge of medical terminology and anatomy, excellent oral and written communication skills

RHIT or RHIA

Anticipated Pay Range: The salary range (or hiring range) for this position is $55,183 - $63,672 / year. The above salary range represents SBUHs good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidates validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.
