The mission of Food Pharmacy is to increase food access for food insecure families while providing support to identify and address broader social determinants of health. Through this program, eligible families will receive a fresh food box subscription along with support from a Resource Navigator to connect them to community programs to address broader social needs. The Food Access Coordinator will coordinate program operations including but not limited to maintain and manage dry goods pantries at all pantry locations, act as an onsite contact and lead for produce markets, recruit and serve as the primary contact for program volunteers, prepare reports and communicate with leadership regarding program operations and advocacy. The Food Access Coordinator reports to the Food Pharmacies Manager. What You Will Do: Follow up with eligible patients for program enrollment, data collection, and the provision of resources Act as the point of contact for enrolled participants to troubleshoot issues and provide emergency resources, with the assistance of the Food Pharmacy Manager Prepare reports and communicate with leadership regarding program operations and advocacy Work with families to develop a mutually agreed upon action plan to follow through on referrals and recommendations Act as the on-site Food Pharmacy contact for your assigned Food Pharmacy Act as the resource navigator for existing and new pharmacies Work with internal and external partners to coordinate food deliveries Liaise with internal partners to identify appropriate programs for patient referrals Develop and maintain external partnerships appropriate for patient referrals Maintain accurate and timely documentation of patient encounters in EPIC and other tracking systems Utilize tracking systems to ensure patient outreach efforts are consistent with program guidelines Responsible for stocking pantries at assigned pharmacy Maintain and manage dry goods pantries at all pantry locations Act as an onsite contact and lead for Produce Markets Recruit and serve as the primary contact for program volunteers Prepare reports and communicate with leadership regarding program operations and advocacy Education Qualifications: High School Diploma / GED - Required Bachelor's Degree Social Work - Preferred Experience Qualifications: At least two (2) years experience in the health services field - Required At least two (2) years of experience in health services, peer navigator role, or community setting providing similar support services - Preferred Previous counseling experience - Preferred Previous experience in a pediatrics setting - Preferred Skills and Abilities: Basic knowledge of HIPAA regulations Basic knowledge of medical terminology Familiarity with electronic medical records systems (EPIC) (Preferred proficiency) Basic proficiency with office software (Microsoft Office) including word processing and spreadsheet software (Word, Excel) (Required proficiency) Strong verbal and written communications skills (Required proficiency) Strong customer service skills (Required proficiency) Strong interpersonal skills (Required proficiency) Ability to maintain confidentiality and professionalism (Required proficiency) Ability to work independently with minimal supervision (Required proficiency) Ability to convey complex or technical information in an easy-to-understand manner (Preferred proficiency) Ability to collaborate with stakeholders at all levels (Required proficiency) Ability to be flexible and adaptable to change (Required proficiency) Ability to drive/travel to multiple facilities/locations as needed (Required proficiency) To carry out its mission, CHOP is committed to supporting the health of our patients, families, workforce, and global community. Salary Range: $51,730.00 - $64,660.00 Annually
