Clinical Lead, Care Management

Job Summary:
Assumes the responsibility for providing daily clinical and administrative support to the Enabling Services Care Management team to maintain daily operations and integrated service delivery. Seeks to prevent illness, injury, disability, and early mortality across the entire population with a particular focus on populations at greater risk of poor health. Collaborates with leaders, clinicians, and staff across VHP, as well as external stakeholders, community agencies, and healthcare facilities to facilitate care management services across the spectrum of care. Serves as the subject matter expert of the care management team, supporting best practices, monitoring of existing workflows and initiatives, and implementation of new programs or initiatives.
Education: Bachelor's degree in nursing required.
Experience: 5 years chronic disease management experience working in a care management capacity. Demonstrated leadership skills required.
1 year of supervisory or lead experience preferred.
Licensure :
Registered Nurse in the State of Pennsylvania
Certifications : CPR
Special Skills: Ability to plan and organize time and resources efficiently. Ability to lead confidently with strong decision-making and influencing skills. Ability to think logically and sequentially and understand redesign concepts and ability to identify opportunities for improvement. Essential Functions:
Provides daily clinical support to the Care Management team, serving as the point of contact for patient care related needs or questions. Escalates to Enabling Services leadership as needed. Helps to develop, implement, and evaluate best practices in Care Managements programs and services, in collaboration with Enabling Services Leadership team. Helps to champion new procedures, workflows, or initiatives involving the Care Management team. Develops educational materials such as PowerPoint presentations or formal written procedures to provide training to colleagues. Ensures care management programs and projects are evidence-based and tailored to local communities. In collaboration with leaders throughout Valley Health Partner's Primary Care and Specialty practices, develops strategic plans related to the Care Management of designated population. In collaboration with Enabling Services leadership, coordinates, and helps to facilitate Care Management team meetings, and assists in execution of required follow-up items to maintain/improve Care Management operations. Supports direct patient care needs, including referrals and/or practice coverage. Responsible to provide care management coverage across the health center. Involved in recruitment and hiring of new Care Managers. Onboards and trains new Care Managers, including completion of clinical competency assessment and development of training tools. Utilizes multiple data reporting tools to monitor, evaluate, and address population health needs unique to Valley Health Partners. Works in partnership with internal and external community resources to ensure that both patient needs and clinical outcomes are met. Monitors the overall daily performance and participation of the team in quality improvement initiatives. Uses effective and appropriate communication styles with patients, visitors, staff, leaders, clinicians, and colleagues. Models the qualities of servant leadership to inspire a sense of shared mission and values among the team and foster a collaborative work environment. Maintains knowledge, skills, and certifications to ensure expertise. Other duties as assigned by leadership. Benefits:
