Hospice Admissions Coordinator

Key Responsibilities: Serve as the primary point of contact for new patient referrals Initiate outreach to patients and families within 1 hour of referral Coordinate all admissions within the company's 3-hour commitment window Verify insurance benefits, payer source, and secure authorizations for hospice care Communicate pending admissions to appropriate clinical and admission staff. Collaborate with clinical teams, billing coordinators, and external referral sources Enter accurate patient data and documentation into the electronic system Participate in morning stand-up meetings, marketing activities, and outreach including seeking new referral sources and developing strategies for company growth Prepare marketing reports regarding referral statistics.
