Position SummaryThe primary purpose of your position is to assist with planning, organizing, developing and directing the overall operation of the Wellness Department and to ensure the delivery of the highest quality assisted living services focused on maximizing residents' individual capabilities in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Executive Director.This is a full-time, exempt position, which often requires work beyond normal business hours, and occasional weekends and holidays. This position requires the incumbent to be on-call 24 hours a day, 7 days a week and is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).Essential Functions and ResponsibilitiesEnsure the CAPLICO Core Values are adhered to at all times.Assist with planning, developing, organizing, implementing and evaluating resident care.Ensure that all residents receive quality care and maximize resident's individual capabilities.Ensure that resident's rights are well established and maintained at all times.Ensure an environment of teamwork and cooperation between Wellness department employees/other departments.Maintain the confidentiality of all pertinent resident care information to ensure resident rights are protected.Assist in the development and implementation of departmental policies and procedures.Ensure effective, efficient and adequate staffing of the resident care department by creating a work schedule that meets the needs of residents 24/7, while being mindful of staffing budget.Ensure that care interventions meet the personal, physical and cognitive needs of each resident as well as maximize his/her self-care capacities, identify, independence, choice and opportunity for social interaction.Ensure that all applicable Human Resources policies and procedures and State and Federal employment laws and guidelines are followed by all resident care department employees. The ability to work in a constant state of alertness and safe manner is essential to the performance of this position. Ensure that all Caregivers are appropriately oriented, onboarded, and trained, according to Facility policy and State regulations. Ensure all caregiving staff complete required training and in-services according to established timeframes.Ensure caregiving staff are routinely trained in skills relevant to proper resident care, and to routinely audit and assess such skills to ensure they meet facility standards for resident care.Oversee monitoring system to ensure licensed care staff keep licenses current and in good standing with respective licensing authority.Ensure employee and resident health and safety programs are implemented to provide a safe environment.Actively maintain cohesive public and professional relationships with residents, families, physicians and the community.Maintain ongoing education as it relates to maintenance of your professional skills and, where applicable, licensure as a nursing professional.Work with Executive Director on Wellness department budgeting and spend downs; ensure adherence to established department budget while also ensuring that quality of resident care is not affected.Work with Executive Director to promptly investigate and properly document any allegations of abuse or neglect. Assure prompt notification to resident's physician, family members, and responsible parties.Monitor and review resident incidents and identify patterns that may require preventive intervention.Ensure resident care staff is properly educated on and utilizes infection control practices that protect staff, residents and families.Periodically visit all Caregiver shifts to monitor performance of staff and to ensure quality care is being provided to residents.Complete resident assessments for resident level of care and appropriate placement.Assist with new resident orientation and transition to community.Meet with residents regularly to encourage communication and gain understanding of their needs. Conduct daily rounds to review performance of Wellness staff.Continuously prepare and ensure readiness for state survey; conduct follow-up to ensure corrective actions are appropriately completed. Provide hands-on care as staffing needs require (and licensure allows).Conduct visits with residents during hospitalizations and short-term rehabilitation admissions.Maintain a resident waiting list in accordance with established policies and procedures.Keep current on Medicare/Medicaid provider regulations for skilled nursing facilities.Maintain daily and monthly census report as directed.Assist with discharge planning and move-outs.Ensure compliance with established procedures when a resident expires. Other duties as deemed necessary and appropriate, or as may be directed by the Executive Director.Supervisory ResponsibilitiesExercises administrative and functional supervision over Wellness Department employees.Work EnvironmentWorks in office area(s) as well as throughout the community/facilityMoves intermittently during working hours.Is subject to frequent interruptions.Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.Is involved in community/civic health matters/projects as appropriate.Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis viruses.May be subject to the handling of and exposure to hazardous chemicals.Physical Demands and Sensory Requirements(With or Without the Aid of Mechanical Devices)Must be able to move frequently throughout the workday.Must be able to cope with the mental and emotional stress of the position.Must be able to see and hear or use prosthetics that will enable the senses to function adequately to ensure that the requirements of this position can be fully met (e.g.: accurately read measurements on resident related equipment such as thermometers, monitors, gauges).Must be able to function independently, have personal integrity, flexibility and the ability to work effectively with residents, employees, visitors and support agencies.Must be in a good general health and demonstrate emotional stability.Must be able to relate and work with the disabled, ill, elderly, emotionally upset and at times, hostile people within the facility.Must be able to lift, push, pull and move a minimum of 50 pounds.Must be able to assist with the evacuation of residents.Specific RequirementsPossess the ability to make independent decisions when circumstances warrant such action.Possess the ability to deal tactfully with personnel, residents, visitors, and the general public based on whatever maturity level they are currently functioning.Ability to be calm and level-headed in emergencies.Well groomed, professional and possess ability to work harmoniously with other personnel.Ability to seek out new methods and principles and be willing to incorporate them into existing admission practices.Follows written and oral instructions.Knowledgeable in the use of computers, e-mail, spreadsheets and document word processing.Maintain patience, tact, cheerful disposition and enthusiasm, as well as ability to handle residents, staff, and visitors, based on whatever maturity level they are currently functioning.Ability to travel for training and work related activities.Minimum Education and Experience RequirementsMust possess, as a minimum, a graduation certificate from an approved LPN/LVN program; Nursing Degree from an accredited college or university is preferred.Must have current CPR certificationIf required by your state of residence, must possess a current, unencumbered, active license to practice as a Nurse.Must possess a current First Aid certification.Prefer, as a minimum, 2 year(s) of experience as a supervisor in a healthcare setting; hospital (geriatrics), skilled nursing or assisted living facility.The employer for this position is stated in the job posting.
