Our client, a Non-Profit Healthcare Organization in Granby CT, is in need of a Nurse Coordinator for a contract to hire position. Shift for this role is Monday - Friday, flexible schedule (40 hours per week). Weekend Call - every 6 to 8 weeks. $300 stipend to take call.
$4k Sign-on Bonus upon conversion to full-time employee.
Nurse Coordinator Minimum Requirements:
Current Connecticut Registered Nurse license; BSN, professional nursing experience Current Connecticut driver's license: Will need to travel between the 3 homes you are coordinating for in the area Knowledge of health and needs of individuals with disabilities; skills and ability to plan and implement ways of meeting these needs in a community setting. Knowledge of DDS/DPH regulations preferred Knowledge skills, and ability to assess and document the health needs of participants. Ability to organize and maintain accurate records. The ability to teach health related issues. Knowledge and ability to maintain a safe and hygienic environment treating information with confidentiality. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. Experience working in Group Home environment or with community nursing (non-hospital) required.
Nurse Coordinator The Group Home Nurse Coordinator ensures the provision of all health services for program participants and follows the directions of medical providers to maintain participant's health and well-being, in keeping with nursing standards, DDS/DPH regulations and corporate mission. The Group Home Nurse Coordinator works closely with participants, parents, guardians, supervisors and department heads, direct support staff and other employees, labor union representatives, behaviorist, SLP, PT, Psychiatrists, doctors, state and federal regulators, vendors, suppliers
