Overview

We are seeking a compassionate, dedicated professional to provide comfort, support, and dignity to patients nearing the end of life. The Hospice Social Worker will play a critical role in addressing psychosocial needs and coordinating care with the interdisciplinary team to support patients and their families.

Responsibilites

• Assess the psychosocial status of patients and families/caregivers related to the patient\'s terminal illness and environment; communicate findings to the registered nurse and other interdisciplinary team members
• Conduct psychosocial assessments in the patient\'s identified residence and assist in safety planning when needed
• Perform social evaluations, including family dynamics, caregiver abilities, communication patterns, and risks related to suicide, neglect, or abuse; plan interventions accordingly
• Counsel patients and families on stress and coping difficulties; provide crisis intervention when necessary
• Educate the interdisciplinary team on culturally related needs, including communication, family roles, space, and taboos
• Maintain clinical records for all patients referred to social work
• Educate patients and families on advanced directives and assist in preparation
• Provide information and referral services for patients and families regarding practical and environmental needs
• Serve as liaison between patients, families, and community agencies
• Maintain collaborative relationships with organization personnel to support patient care
• Develop contacts with public and private agencies as resources for patients and staff
• Participate in the development of the individualized plan of care with the patient and family; attend interdisciplinary group meetings and help the team recognize psychosocial effects on symptoms
• Assist physicians and team members in understanding social and emotional factors related to health problems and end-of-life issues
• Actively participate in quality assessment and performance improvement activities
• Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities
• Perform other duties as delegated by the Clinical Supervisor

Requirements

• Must have a master’s degree in social work from an accredited university
• If you hold an undergraduate B.A. Degree in Social Work, you are eligible to apply. The hire rate of pay will be based on degree type
• Minimum of two (2) years of documented supervised experience in health care; hospice experience preferred
• Understanding of hospice philosophy and issues related to death/dying
• Certified Hospice and Palliative Social Worker is desirable
• Flexibility in work hours and ability to travel throughout the Bristol Hospice service area
• Willingness to maintain knowledge of information systems and applicable software programs
