The Social Worker – Medical Respite Program provides trauma-informed, person-centered support to young adults experiencing housing insecurity and medical challenges, particularly those facing maternal health complications or undergoing cancer treatment. This role is part of a new initiative launched in partnership with New York State under the Medicaid waiver, offering medical respite housing and supportive services to up to 10 patients and their caregivers or family members at a time.
The Social Worker – Medical Respite Program is responsible for conducting intake assessments, developing individualized care plans, facilitating service navigation, and monitoring participant progress. This position reports to the Medical Respite Program Manager and involves collaboration with internal team members and external partners to ensure coordinated care. The Social Worker may support families staying in the organization’s core housing programs and contributes to a culture of teamwork, empathy, and high-quality care across the Family Support team.
The salary range for this role will be 60K-70K. The schedule is Monday through Friday, 9 AM – 5 PM with some flexibility and weekends as needed.
Responsibilities
- Case Management: Conduct SDOH assessments upon intake and develop individualized service and support plans.
- Provide trauma-informed, strengths-based support and care coordination.
- Facilitate service navigation across medical, housing, mental health, benefits, and community support systems.
- Monitor progress, adjust care plans as needed, and maintain regular contact with participants during their stay.
- Advocate for participants’ needs within health, housing, and social service systems, helping to reduce barriers to access.
- Identify and address systemic gaps or inequities impacting participant well-being, escalating concerns when appropriate.
- Collaboration & Team-Based Care: Collaborate with internal staff and external partners to ensure coordinated, holistic care across disciplines.
- Contribute to team case reviews, multidisciplinary planning, and continuous quality improvement.
- Partner with on-site and off-site social workers, medical professionals, housing navigators, and support staff to ensure wraparound support.
- Documentation & Compliance: Maintain accurate case notes, assessments, and reporting in accordance with Medicaid and organizational standards.
- Support data collection and reporting for funders, audits, and program evaluation.
- Uphold HIPAA and all relevant confidentiality and regulatory standards.
- Program Support & Coverage: Participate in a 24/7 staffing coverage model, which may include occasional evening or weekend on-site support.
- Serve as backup support to the broader Family Support team as needed.
- Assist with participant transitions, crisis response, and discharge planning.
Qualifications & Competencies
- MSW (Master of Social Work) from an accredited institution.
- LMSW or LCSW preferred.
- Minimum 2 years of relevant experience with individuals facing housing insecurity, maternal health challenges, or chronic illness.
- Demonstrated understanding of trauma-informed care, housing navigation, and Medicaid programs.
- Strong interpersonal, communication, and organizational skills.
- Commitment to equity, dignity, and culturally responsive care.
- Preferred: Experience in a residential or respite care setting; familiarity with NYC social service and housing systems; bilingual abilities (English/Spanish or other relevant languages) a plus.
