Overview

Social Worker role at DaVita Kidney Care. You will be part of a patient-centered care team in an outpatient setting, supporting patients through the physical, mental, emotional and financial aspects of End Stage Renal Disease.

Responsibilities

• Build meaningful, long-term relationships with patients and their families in an outpatient setting.
• Support and advocate for patients as they transition to and manage dialysis-related care.
• Help patients understand their rights and responsibilities and guide them through relevant programs and resources.

Requirements

• Meets all state regulations to practice in a dialysis setting.
• Master’s degree in Social Work (MSW) required.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills (MS Word, Excel, PowerPoint, Outlook) and proficiency with DaVita applications within 60 days.

• Two years of experience as a social worker in a healthcare setting
