Travel Physical Therapist - Home Health

American Traveler seeks a Physical Therapist with at least 1 year of home health experience for a day-shift Home Health position requiring Oasis and Epic documentation skills.Job Details•Home health setting serving Medford, OR and surrounding areas,•5x8 hour day shifts, 8:00 am to 4:30 pm, no weekends,•Typical caseload is 1:1 with up to 6 patient visits per day and a 30 point productivity standard,•No floating between units required for this assignment,•No on-call or weekend coverage required,•Uses EPIC and Oasis for documentation; experience with both is required,•Mileage reimbursement offered at IRS standard rates,•Colorful scrubs permitted,•Parking is free,Job Requirements•Active Physical Therapist license required if applicable,•Minimum 1 year of recent skilled home health experience required,•Current BCLS certification required,•Proficiency in Oasis and Epic documentation required,•Reliable personal transportation required for patient visits,•Must be able to handle a caseload with up to 6 patient visits per day within a variable radius up to 50+ miles,•Pending license accepted,•Minimum of 2 supervisory references covering 1 year within the last three years in home health,•SSN and DOB required for consideration,Additional Information•Primary responsibilities include providing in-home physical therapy assessments and treatments while documenting in Epic and Oasis,•All travelers are required to float between facilities within the state if needed,•Locals considered ,local•within a 50-75 mile radius and accepted at the same rate,•Not accepting first-time travelers,•Modules must be completed prior to start and are non-billable, typically taking about 16 hours,•Minimal to no RTO is approved, and all time-off requests must be addressed before starting,•Former permanent staff must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the facility,•Travel team supports scheduling to minimize drive and windshield time,•Communication devices may be provided if required for assignment
