Hourly Pay Range:
$40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
Position Highlights: Position: RN Patient Care Spine Navigator
Location: Edward Hospital - Naperville, IL and Elmhurst Hospital - Elmhurst, IL
Full Time: 40 hours/week
Hours: Monday - Friday, 8:00am - 4:30pm (business hours)
What you will do: Functions within a multidisciplinary team of physicians, office operations and acute care services team members to ensure that patients receiving spine care receive quality and comprehensive services throughout the care continuum. Serves as the clinical resource with expertise in spine care management. Collaborates with physicians, healthcare professionals, and administration to identify opportunities to improve patient experience and care delivery. Analyzes outcomes data and reviews current literature to recommend process improvement initiatives. Serves as liaison across the healthcare system and community regarding services provided for this patient population. Promotes collaboration across ambulatory and acute care spine program stakeholders. Develops and delivers patient education and serves as a resource for patients and their care partners.
What you will need: Bachelor's Degree in Nursing
Minimum three years of RN clinical experience
Current State of Illinois Registered Nurse licensure
Current CPR Certification (BLS) issued by American Heart Association or American Red Cross
Education, Experience and/or Skills Preferred : Minimum five years of RN clinical experience
Experience in delivering care to spine patient populations
