We are looking for an experienced and motivated Lead Care Manager to join our network of clinics serving Kansas City, MO and the surrounding areas. The RN Care Manager requires a strong foundation in healthcare management, proven leadership abilities, and a deep commitment to enhancing patient outcomes. The Lead Care Manager will be instrumental in guiding care management initiatives, supporting targeted interventions to reduce hospitalizations and emergency visits, and collaborating closely with clinical teams to optimize operations and ensure the delivery of high-quality care. Develop and implement policies, procedures, and workflows to enhance clinic efficiency and care coordination. Oversee patient assessments, care planning, and interventions to maintain compliance with healthcare standards. Collaborate with multidisciplinary teams to optimize patient outcomes and streamline service delivery. Monitor performance metrics, analyze data, and prepare reports to identify improvement opportunities and inform strategic decisions. Ensure regulatory compliance and uphold organizational standards across all care management activities. Facilitate team meetings and training sessions to promote continuous learning and best practices. Qualifications Before we go any further, we do have some deal-breakers. You must have: Current state Registered Nurse or LPN license as required by location. Registered Nurse experienceTwo years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past three years. LPN experiencea minimum of five years' primary care practice experience with adult medicine and pediatric patients in a primary care/family practice setting. Care management experience preferred.
