The

Director of Patient Care Services (DPCS)

provides strategic and operational oversight of all patient care services and associated personnel across our Home Health programs. This critical leadership role ensures the coordination and delivery of high-quality home health care that aligns with patient needs, agency standards, and regulatory compliance.

The DPCS oversees patient assignments, coordination of care, staff performance, referrals, and clinical operations. This role works in collaboration with the COO to develop and implement agency objectives in alignment with our mission and vision. The DPCS is also accountable for fostering an engaged, competent, and compliant team and is available during all operating hours or designates a qualified alternate.

Key Responsibilities

Provide leadership and supervision to staff. Ensure high-quality, person-centered care through oversight of care planning, implementation, and evaluation. Oversee staff development, engagement, and performance management. Ensure adherence to all relevant policies, regulations, and compliance requirements. Participate in strategic planning and budget development. Serve as a liaison between patients, staff, and leadership. Maintain availability during business hours or ensure coverage by a qualified alternate. Qualifications

Possession of a clear active Registered Nurse (RN) license in California. A registered nurse with a baccalaureate or higher degree in nursing or other health related field with 3 years' experience within the last five years in home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity or a registered nurse with four 4 years experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisor capacity. Strong critical thinking and independent judgment skills. Effective leadership, collaboration and team coordination abilities. Proficiency or ability to learn computer systems and software programs relevant to the role. Benefits
