We are hiring for a Supported Employment Program (SEP) Specialist at Leading By Example LLC!

The Supported Employment Program (SEP) Specialist is responsible for facilitating Targeted Case Management (TCM) services for adults. The SEP Specialist will maintain a minimum caseload of 10 TCM clients and will provide 1-5 service hours per TCM client per month. The SEP Specialist must provide a minimum of 50 TCM billable services per month. The SEP Specialist is required to participate in Clinical Supervision sessions twice per month. The SEP Specialist will maintain a client caseload in conjunction with the Supported Employment Program of up to 15 clients. The SEP Specialist will play a vital role in assisting clients individuals by connecting them with vocational services and achieving their employment goals. The SEP Specialist's primary focus will be to provide comprehensive support to clients throughout the job search process, helping them secure and maintain meaningful employment opportunities.
This is an hourly full-time position. The rate is $22/hour. This is a Community Based position so you are not required to come into the office but M-F with daytime availability is essential. You will be required to meet clients face-to-face so you must have your own reliable transportation with an active drivers license and insurance.
TCM Services: Provide each assigned client with their prescribed number of Targeted Case Management (TCM) Facilitate intakes and completes assessments to establish service goals and care plans (treatment plans). Provide support and referrals to community resources, including housing; link to entitlements/benefits, and behavioral and somatic health care providers; and assist in developing social support systems and monitoring of engagement of services. Implement interventions identified on behavior and/or rehabilitation plans. Model positive behaviors and character to clients and the community at all times. Adhere to the terms, rules, regulations, and policies of LBE. Assist with the development and implementation of individual and treatment plans. Review the treatment goals and interventions with the client and obtain required signatures each authorization period (bi-annually) at minimum. Provide emergency crisis intervention and conflict management to identified clients. Track, monitor, and document the client's progress in achieving established goals and utilization of community resources. Also, utilize designated documentation tools. Ensure clients receive assistance and support with physical and somatic health management needs which includes understanding and managing medication. Educate and support clients and families in implementing effective behavioral intervention strategies at home and in the community. Develop and implement therapeutic recreation, cultural enrichment, and independent living skills activities. Facilitate family and community support. Complete progress notes and monthly summaries for TCM on a weekly and monthly basis. Complete critical incident/hospitalization reports and submit them to the supervisor within 48 hours. Submit all documentation as it pertains to progress notes, monthly summaries by specified deadline. Collaborate with therapists, case managers, rep payee, doctors, psychiatrist, DSS case manager, private landlords, realtors, property managers, HCAA, Harford County Health Department, United Way Homeless Services, etc. Assist clients with applying to SOAR SSI/SSDI on a bi-annual basis. Attend therapy appointments for intensive clients in crisis. Attend further training as required for LBE. Participate in LBE supervision. Vocational Services:
Support SEP clients to be able to choose, obtain, and maintain employment, and make vocational decisions and career changes Support SEP clients vocational plan throughout clients employment, including periods of unemployment Provide ongoing assessment and planning with the SEP client and, when appropriate, the clients family or natural supports Conduct job coaching and vocational training, which may include training in job tasks and responsibilities. Educate SEP clients on the financial impact of employment on disability benefits, entitlements, and other benefits, access to such Social Security programs as Plan to Achieve Self-Sufficiency and Impairment-Related Work Expense and the individual's rights and responsibilities under such programs, access to additional benefits and other resources, and benefits and risks of disclosure to the employer of the individual's disability. Market to industries in the community and develop close cooperation with industries, unions, and other community resources related to employment opportunities Regularly survey the local business community and develop an ongoing marketing strategy to increase employment opportunities for clients Referral for services the SEP clients needs, which are not provided by the program Promote placement in appropriate and safe work environments Assist SEP clients with activities directed toward the development of skills, including, but not limited to personal hygiene and grooming specific to the employment environment, effective communication with supervisor and coworkers, and transportation to and from work Encourages the SEP clients to use natural supports and develop an employment-related support system through relationships with coworkers, supervisors, and others in the work environment Have at least two face-to-face contacts a month with the SEP client. Provide vocational promotion services to SEP clients, offering guidance and support in career exploration, skill development, and job retention. Assist SEP clients in identifying and addressing barriers to employment, such as transportation issues, lack of necessary certifications, or social skills development. Offer guidance in resume writing, interview preparation, and job application techniques. Assist SEP clients in completing job applications, preparing for interviews, and providing on-the-job support during the initial stages of employment. Documentation:
Complete intake packets for assigned clients. Complete care coordination notes for each visit with clients. Complete Vocational and Needs Assessments and treatment plans for clients when required. Complete critical incident/hospitalization reports and submit reports to your supervisor within 48 hours. Submit all documentation by specified deadlines and communicate any difficulties in doing so. Coordination:
Collaborate with each TCM client's therapist on a monthly basis. Collaboration efforts may be face to face or by phone. Collaborate with each TCM client's various treatment providers on a regular basis. Conduct at least one contact per month with the SEP clients vocational industry to receive information regarding the SEP clients performance on the job Supervision:
Participate in supervision sessions with Clinical Supervisor and Service Manager twice per month. Participate in supervision with the TCM/SEP Manager once a month Attend a general staff meeting on a quarterly basis. Attend clinical training sessions when required. Attend Programmatic Trainings through the State of Maryland as required Qualifications:
Bachelor's Degree preferred 2 years' experience providing direct services to adults with emotional and behavioral challenges preferred, including vocational experience Reliable and current auto insurance (State minimum for Maryland). Valid Maryland Driver's license. CPR/ First Aid Certification. Benefits (including but not limited to):
Medical, Dental, and Vision insurance beginning on the 1st of the month following the 60th day of employment. FSA/HRA available. LBE pays for employee life insurance. New employees may utilize PTO days after their 90-day Introductory Period. The amount of PTO new employees are front-loaded at the beginning of each calendar year is based on the length of service determined by the Organization: After 1 year employees will receive 15 days of PTO, after 5 years you will receive 20 days of PTO, and after 10 years 25 days of PTO.
