Job Title: Medical Assistant - Population HealthLocation: East Valley, ArizonaOverview:We are seeking a detail-oriented and proactive Medical Assistant to support our Population Health initiatives. This role is designed to enhance patient care coordination and improve compliance with wellness and quality programs. While this position does not involve direct patient care, it plays a vital role in supporting primary care teams through chart reviews, care gap closures, and patient outreach.Key Responsibilities:Review patient charts to identify screening needs and care coordination opportunities.Execute standing orders for screenings and quality gap closures as directed by primary care physicians.Prepare charts prior to office visits to support value-based care initiatives.Manage in-network referrals and coordinate follow-up visits for transitions of care.Conduct outreach to schedule appointments for wellness visits, well-child checks, and cancer screenings.Transfer quality care gap data from clinic records to population health platforms.Address payer care gaps and support high-risk patient management.Collaborate with primary care teams and clinical performance consultants.Maintain transparency regarding training, credentials, and experience.Work cooperatively with office staff and support teams.Perform additional duties as assigned.Required Skills & Qualifications:Proficiency with electronic health records (EHR) and scheduling systems.Experience with EMR platforms and patient outreach.Minimum of 3 years' experience in population health or referral coordinatorMedical Assistant certification (CMA) or diploma from an accredited program.Strong verbal communication and organizational skills.Preferred Skills:Ability to analyze data and manage tasks to meet performance goals.Basic understanding of healthcare metrics such as ALOS, readmission rates, and care gaps.Work Environment:Based out of a central hub in Arcadia.Mondays are office-based; Tuesday through Thursday involves travel to provider offices across East ValleyFridays may be remote or clinic-based depending on operational needs.Pay and BenefitsThe pay range for this position is $22.00 - $28.00/hr.Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
