Job Summary

Serves as a point of contact for referring physicians, patients, and caregivers to provide resources andpatient education while performing a variety of other clinical and non-clinical related responsibilities of patient care within the OP Women's Breast Center department. The Breast Imaging Nurse Navigator will assist with patient appointments,imaging procedures, and specialty consultations while delivering breast related diagnosis and ensuring patients understand their diagnosis, treatment options, and resources available.

Performance: Position Specific Essential Functions Adheres to Standards and recommended practices for specific nursing service area(s). Provides a safe supportive environment for surgical/procedural patients and expected practice behaviors based on the nursing process of assessment, planning, implementation of nursing interventions and evaluation. Documents appropriately in the patient record. Uses professional judgement on pertinence of documentation. Ability to identify emergency situations and uses sound judgement in responding to emergencies in a calm and efficient manner. Works collaboratively with all team members to include staff, physicians, ancillary departments and patient/families. Demonstrates a positive attitude and good communication. Understands that attendance affects team morale and confidence. Demonstrates reliability and punctuality on attendance. Consistently present and ready to begin work activities at start of shift, stays within established break periods, arrives on time for meetings and leaves work area promptly at the conclusion of the shift to reduce potential distractions. Schedules personal appointments or requests adjustment to schedule with least amount of disruption to service delivery. Provides prompt notice for unexpected absences. Schedule breast biopsy procedures, breast MRI's and follow up consultation appointments. Reviewing patient allergies and blood thinners. Understand pre-authorization and or estimate process for breast imaging procedures. Breast Navigator specific Duties: Provide educational support and consistency through and after initial diagnosis; Help arrange consultations for oncology/surgery; schedule any additional imaging or testing for the patient; understand the continuum of care required to assist breast care and cancer patients; Deliver breast cancer diagnosis in-person to patients; provides resources for any barriers in care; Coordinates and collaborates with other providers for continuum of care; Enter pathology results; Maintain a registry of results; Resolve imaging and pathology reports; Maintains proper and efficient flow of exams, reports, charges, and denials. Enters lab orders on behalf of the radiologist and educates patients on the process for genetic testing. May be asked to assist radiologist during and after biopsy procedures, prepping patient, obtaining/taking vital signs, going over/review patient allergies and history. Provide after care instructions education, hold pressure and apply bandage over biopsy site. Start IV's if procedure requires. Follow up with patient post procedures. Maintains and tracks outcomes for collection of data with Oncology providers Collaborate with other nurse Oncology navigators to communicate patient barriers to care. Ability to adjust to a varying workload; assume assignments in a positive manner; attend required meetings and in-services; maintains department competencies and certifications Performs other duties as assigned.
