Job Summary
The Division of Hospital Medicine - Michigan Medicine is seeking board certified/board eligible Internal Medicine physicians interested in becoming part of a large hospitalist group. This group consists of 90+ hospitalists in addition to 30+ advance practice providers serving across 10 unique services providing a variety of care models. We are interested in candidates who will complement our culture of collaboration, compassion, and professionalism while being an advocate for the patient, as well as serving the needs of the hospital. ACLS certification is required.
30% of your overall effort will be devoted to overseeing the day-to-day clinical operations of the Hospital Care at Home (HCAH) Program.
What You'll Do
The Medical Director of Hospital Care at Home (HCAH) serves as the physician leader for the HCAH unit, which is an innovativecare delivery model that offers eligible patients the opportunity to have their inpatient-level hospital care needs met from the safety and comfort of their own homes, enhancing both patient experience and clinical outcomes. HCAH is critical to organizational goals around capacity management and improving patient access. This position requires developing and implementing transformative solutions, innovating clinical best practices, and driving exceptional patient-centered care.
The medical director is responsible for advancing the clinical vision and leading the HCAH team to achieve operational excellence, exceptional clinical performance, patient experience, and outcomes while promoting clinical innovation and the overall success of HCAH. This role will work closely with the HCAH Administrative Director, nursing leadership, and vendor (Medically Home Group/DispatchHealth) leadership team. The medical director oversees physicians in the HCAH virtual command center and ensures provision of high-quality virtual inpatient care. The medical director will also work closely with the patient acquisition team, comprised of advanced practice providers, who are responsible for intake and admissions to HCAH.
The medical director is expected to demonstrate exemplary clinical, leadership, strategic planning, critical thinking, patient relations, organizational, and time management skills. The position requires a leader who is passionate about transformative virtual care solutions, innovating clinical best practices, and driving exceptional patient-centered care. The medical director will collaborate with interdisciplinary teams including advanced practice providers, nurses, physicians, pharmacy, administrative staff, clinical departments, Health Information Technology Services (HITS), and others to meet all performance goals related to HCAH.
In addition to a leadership role, the Medical Director will provide direct clinical care as a virtual hospitalist on the service distinct from the above leadership effort.

Responsibilities *
Performs all clinical and administrative duties expected of a medical provider.
Serves as the dyad partner to the HCAH administrative director for day-to-day operational management of the virtual unit; also works in collaboration with nursing director and Medically Home Group/DispatchHealth (MHG/DH) operational leadership.
Drives clinical model design, policy, HCAH eligibility, and strategic plan. Leads all aspects of clinical operations and partners with administrative director and project management on leadership of HCAH governance to coordinate HCAH initiatives and priorities.
Supervises HCAH virtual hospitalist team, which includes evaluating medical provider performance, chart review, mentorship, and providing regular feedback to provider team on clinical performance and other indicators.
Meets with individual providers as needed to assess satisfaction and offer resources and support as needed. Identifies and assists providers in need of performance improvement, quality of care assessment, general support, or other training.
Ensures appropriate provider staffing levels and plans for staffing and future recruitment needs in partnership with the Division of Hospital Medicine. Coordinates medical provider and advanced practice provider schedules.
Leads provider recruitment, including hiring and onboarding/orientation of new medical providers with a focus on identifying physicians and advanced practice providers who are a good fit for the unique HCAH model.
Assists with evaluation and completion of follow-up on safety event reports. Escalates areas of concern as needed and appropriate and follows through to resolution.
Develops, updates, and champions clinical guidelines and standards of care, effectively communicating with provider team around new information and changes.
Leads the development of clinical protocols, workflows, and care pathways that will enhance HCAH patient acquisition and support health system goals related to HCAH admission targets.
Maintains HCAH unit-specific service provider documents, HCAH order sets, and other clinical documentation.
Leads change management efforts with clinical teams and leadership to build support for the HCAH model of care delivery and drive referrals to HCAH. This includes physicians, Care Management, Nursing, department/division leaders and patients and families.
Serves as an escalation point for patient acquisition questions and troubleshooting, including individualized follow-up with clinicians who prevent HCAH admissions.
Represents HCAH to internal and external stakeholders, including building relationships with peer organizations and community partners as needed.
Manages regulatory activities and interaction with applicable agencies on the local, state, and federal level, in partnership with administrative director and project manager.
Creates and fosters an environment of teamwork and interprofessional collaboration to ensure an integrated, team-based approach to patient care. Engages all members of the care team in problem-solving, including providers, nursing, pharmacy, and vendor partners to expand and strengthen HCAH.

Licensure & Certification
Active Medical Staff Member
Certification by the American Board of Internal Medicine, American Osteopathic Board of Internal Medicine, American Board of Family Medicine, or American Osteopathic Board of Family Physicians within five years of completion of accredited training and subsequent maintenance of certification.
Advanced Cardiovascular Life Support (ACLS) certification
