Salary: $130,000 - $140,000 per year

Low patient volume for better care and work-life balance
All equipment provided: laptops, tablets, and more
Relocation assistance available
New grads welcome with full training and support
Conduct Medicare annual health assessments in clinic and at patient homes
5-6 patients/day
Perform H&Ps and document in EMR systems
Collaborate with patients’ PCPs to ensure continuity of care
Requirements
Active NP license in Indiana
Comfortable with EMR systems and electronic documentation
Professional, flexible, and passionate about patient care
Strong team player with a compassionate approach
