The Housing Navigation Case Manager (HNCM) will work within a team of case managers to develop and execute sustainable and expeditious housing plans for currently homeless guests. The HNCM will engage with homeless guests to reduce barriers to housing and develop individual housing plans that rapidly lead to safe, affordable housing with supports to sustain achieved housing. The HNCM and team will enter guest information into Pine Street Inn’s HMIS with accuracy and within tight timeframes. The role emphasizes attention to detail, exceptional customer service, and collaboration with other PSI departments to provide integrated service delivery that furthers our mission to eliminate homelessness. The Housing Navigation Case Manager will work from principles including: All people experiencing homelessness can achieve housing stability in permanent housing. Everyone is “housing ready.” Homelessness programs and housing providers must be “consumer ready.” Leverage guests’ strengths and connections to move quickly toward housing. Recognize impact of violence and victimization on development and coping strategies. Employ an empowerment model. Maximize guest choices and control over recovery in a relational collaboration. Create an atmosphere respectful of safety, respect, and acceptance. Emphasize guests’ strengths and resilience. Minimize possibilities of re-traumatization. Strive to be culturally competent and understand each person in their life context. Solicit guest input and involve guests in designing and evaluating services. Responsibilities

Engage with homeless guests to assess needs and develop individualized housing plans. Enter and maintain accurate guest information in HMIS. Collaborate with PSI departments to provide integrated services and remove barriers to housing. Support rapid placement into safe, affordable housing with required supports. Qualifications

Education/Training: Required: BSW, BA or BS in a human service-related field, or equivalent experience (minimum three years in a social service setting) in lieu of a degree. Preferred: Bilingual – Spanish/English and/or Haitian-Creole; counseling or case management certification/training. Knowledge/Experience: Required: at least two years delivering services to a population with addiction and mental health concerns; strong computer/Excel skills; experience creating housing plans with homeless individuals; knowledge of homelessness and housing issues. Preferred: Experience with Motivational Interviewing, Trauma-Informed Care, Harm Reduction, Housing First, Change Theory; experience assisting with budgeting. Job details
