Job Description
The Center for Alcohol & Drug Treatment is looking for a part-time (0.4 FTE) certified Nurse Practitioner or Physician Assistant to work in our Office-Based Opioid Treatment+ (OBOT+) program. The individual in this role would work in OBOT within HDC's Urgent Care Behavioral Health Care Center. You would see patients for a variety of needs related to substance use disorders, and may coordinate care between OBOT+ and other programs. You'll work with an on-site LPN and Patient Access Representative. 

Summary:
The Nurse Practitioner is a key member of the CADT Medical Team and will be responsible to provide medication-assisted treatment for eligible patients.

Essential Duties and Responsibilities:
  • Understand emerging models of healthcare delivery that impacts CADT and its patients;
  • Identify and define new and innovative strategies to achieve business goals and objectives;
  • Identify opportunities to collaborate and develop clinical integration with other healthcare delivery systems to achieve positive and affordable outcomes;
  • Assist with various accreditation and licensing processes critical to CADT;
  • Actively engage stakeholders in the community to both receive feedback, represent CADT interests and services;
  • Promote collaboration to both internal and external stakeholders;
  • Medically manage patients in the OBOT Program;
  • Maintain PMP access and manage delegates;
  • Nursing education and instruction as needed;
  • Collaborate with community physicians, CSAT, social workers internal and external to CADT, pharmacists/toxicologists, and other health professionals for patient care;
  • Patient and family education;
  • Provide biopsychosocial evaluations for adults at substance abuse clinic; and
  • Other specific duties that are mutually agreed on.
Qualifications:
  • A current, unrestricted license to practice as an advanced practice nurse in the state of Minnesota;
  • A minimum of five (5) years’ experience in direct patient care;
  • At least one (1) year of experience providing Medication Assisted Treatment or prior work in a substance use clinic or hospital setting;
  • Holds a DATA waiver for providing Suboxone or is willing to seek immediate certification;
  • Knowledge of managed healthcare systems, medical quality assurance, quality improvement, and risk management;
  • Strategic and innovative thinker with the ability to communicate a vision and drive results;
  • Ability to problem-solve and work collaboratively across disciplines;
  • Experience in case management, disease management, and population health; and
  • Strong written and verbal communication skills;
Preferred Qualification:
  • Experience in behavioral health, substance recovery, or community health preferred;
  • Experience with diverse populations in an urban setting preferred; and
  • Preferred credentialed with insurance companies.
