Job Description
Position requires a Nursing Degree/Licensure (Registered Nurse) and a minimum of five years of experience in the mental health field. Must have excellent clinical, organizational, and interpersonal skills and a valid driver’s license.
Physical Requirements:
· Equipment use including but not limited to computer (including repetitive motions of typing), seeing a monitor, use of a copier, fax, printer, shredder, telephone, and operation of a motor vehicle.
· Sitting for extended periods of time, walking up stairs, balancing, reaching, bending, stretching, standing, walking, and lifting up to 25 lbs.
B. Duties/Responsibilities:
* Will be responsible for Nurse Navigator program including maintaining a client caseload, scheduling appointments to meet with clients in the community and in the office.
* Completing progress notes, assessments, screeners, encounter forms to document all client activities and engagement within the electronic health record.
* Patient Education: Explain medical information in an understandable way, ensuring that clients grasp their diagnosis, treatment plans and potential side effects.
* Care Coordination: Facilitate communication and collaboration with the various healthcare professionals involved in the client’s care, ensuring a seamless and coordinated approach.
* Resource Navigation: Connect clients to relevant supportive services including, financial assistance, community services, support groups and other ancillary supports that assist with meeting treatment goals.
* Psychosocial Support: Nurse Navigators provide emotional support to clients and their families (if relevant) in order to help them cope with the challenges of their diagnosis and treatment. This can be accomplished by attending appointments with clients, coaching clients to advocate and prepare for their appointments and assisting clients when communicating their needs and concerns to healthcare providers and family.
* Treatment Adherence: Work to ensure that clients understand and follow their treatment plans, maximizing the chance of positive outcomes.
* Advocacy: Advocate for clients’ needs, ensuring that they receive timely and appropriate care.
* System Navigation: Assist clients navigate the often-complex healthcare system, scheduling client appointments, coordinating procedures, facilitating access to necessary services.
* Meeting with clients in the office and community to guide patients through the healthcare system.
* Track client progress via treatment plans, vitals monitoring and data collection.
* Represent PMHC clients as an advocate, educator and coordinator in order to help clients understand their medical needs, diagnoses, available resources and treatment options.
* Maintaining internal and external contacts and correspondence with client supports and healthcare professionals.
* May assist or perform Clozaril management duties including the monitoring of vitals as required, and the coordination of blood draws/lab results/pharmacy liaison duties
* May perform Psychiatric Nurse duties including psycho-educative groups, psychiatric medication monitoring, nursing assessments, individual counseling, and all required documentation
* May perform Nursing triage functions in the clinical operation facility. Includes completion of required assessment forms, all other required charting, and the coordination of patient concerns and psychiatric services.
* May perform medication triage services for telephonic requests/concerns regarding medication issues. Coordinates these calls with appropriate psychiatric staff.
* Conducts duties in professional and ethical fashion at all times
* Acts as nursing resource for other departments as needed and available.
* Assist in developing procedure/policy re: nursing services as well as structure to meet requirements of external agencies.
* Metabolic screening of consumers according to requirements of payor or other external agencies requiring.
* Measure vitals of consumers and coordinate care with PCP as required.
* May participate in assisting with sample or indigent medication organization, storage, etc.
* Maintains assigned work area in a clean, organized condition and report any hazard immediately to supervisor.
* Demonstrates a caring, respectful, responsive, and professional manner in all dealings with clients, colleagues, and stakeholders.
