Job Summary:
We are seeking a Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will provide comprehensive care services to patients of all ages and diversities. The ideal candidate will be skilled in diagnosing and managing acute and chronic conditions,
Key Responsibilities:
- Conduct comprehensive physical exams, obtain medical histories, ad assess patient health status
- Diagnose and treat a wide range of acute and chronic illnesses across the lifespan
- Order, perform, and interpret diagnostic tests
- Prescribe medications and therapies in accordance with state regulations and scope of practice
- Develop and implement individualized care plans in collaboration with patients and their families
- Provide preventative care
- Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated patient care
- Maintain accurate and timely documentation in EHR
- Educate patients and families on disease prevention, health maintenance, and treatment plans
- Participate in quality improvement initiatives and adhere to evidence-based practice guidelines
- Stay current with advancements in healthcare through continuing education and professional development
Qualifications:
- Master's or Doctoral degree in Nursing from an accredited program
- Current, unrestricted Family Nurse Practitioner certification (e.g., AANP or ANCC)
- Active Kentucky Nurse Practitioner License
- DEA (preferred)
- BLS/ACLS certification (preferred)
