Responsibilities

JOB SUMMARY / PURPOSE

The Patient Navigator will act as the key non-clinical contact that identifies needs and provides information about services and processes.

ESSENTIAL KEY JOB RESPONSIBILITIES

Guide patients through and around barriers within the healthcare system.

Maintaining communication with patients, families, and health care providers to monitor patient satisfaction.

Direct patients to resources and manage information.

Coordinating screenings and appointments.

Arranging transportation.

Facilitating follow-up services.

Facilitating financial support and helping with paperwork.

Perform other duties as assigned.

Qualifications

Required Education and Experience:

Bachelor's Degree

Two years of related experience in a healthcare environment.

Required Licensure and Certifications:

None

Required Minimum Knowledge, Skills and Abilities:

Strong customer service skills.

Knowledge of medical terminology.

Pay Range

$15.61 - $22.34 /hour
