Type: Full-Time (75 hours biweekly)

Shift: Days

Hours: 9:00 AM - 5:30 PM

Pay: $45.28 - $56.00 per hour depending upon experience

Job Summary

The Social Worker is responsible for providing a variety of direct services, casework, discharge planning, and therapy to patients and their families. Incumbent meets with related departments' medical staff to plan for discharges and other social work interventions and is guided by a thorough knowledge of the psychosocial evaluation process, social work intervention, rules & regulations of the Dept. of Health, Education, Welfare, and the rules & regulations of the hospital.

Responsibilities

  • Completes psychosocial evaluations
  • Completes collaborative treatment planning and implements patient centered services, involving family and/or care partners in the treatment plan process
  • Conducts individual, group psychotherapy as prescribed by the treatment plan
  • Participates in departmental meetings
  • Documents according to clinical standards
  • Develops after care plans for patients upon discharge including linkages to other agencies as appropriate
  • Proficiency in implementing Suicide Risk Assessment
  • Proficiency in implementing Telehealth Services Policy in compliance with Federal and state regulations

Required Skills

  • Masters Degree in Social Work from an accredited school of Social Work
  • Valid NYS LMSW or LCSW
  • Previous Social Work experience in a hospital or E.R. setting required.
  • Experience with Mental Health patients, Assessments, Discharge Planning, and Community Resources
  • Familiarity with ACS and CPS and other legal procedures
  • Background knowledge of CMS, DOH, TJC, and OMH regulations
  • Bilingual English/Spanish preferred
