Senior Patient Care Manager/Clinical Manager

to become part of our team. You will report directly to the Executive Director or Administrator and play a key role in assisting branch leadership to maintain a high-performing hospice program. This includes managing, overseeing, and directing patient care activities to ensure quality delivery of care. You will also supervise, teach, evaluate, and develop team members while ensuring compliance with company policies and state and federal regulations. Prepare for and facilitate the Interdisciplinary Group (IDG) meetings, including patient presentations, coordination of minutes, review of patient charts, and soliciting input from care team members. Manage clinical and administrative recordkeeping functions, such as documenting referrals, verifying Medicare patient eligibility, and handling durable medical equipment pickups. Direct and assign tasks for clinical associates, schedule visits, and ensure timely completion of patient visits. About You: Graduate of an accredited nursing school; a bachelor's degree in nursing is preferred. Current Registered Nurse licensure in your state of residence. Minimum three years of direct patient care experience and two years in a hospice or home health setting. Knowledgeable in hospice principles, industry standards, regulations, and compliance (e.g., Medicare, Medicaid, JCAHO, ACHC). Comprehensive understanding of managed care principles and regulatory guidelines. Proven ability to provide program management and oversight, ensuring quality and compliance.
