Clinical Role:

    • Permanent job opening
    • Seeking a nurse practitioner (NP) or physician assistant (PA) to work full-time (5 days per week) doing primary care rounding in a skilled nursing facility in the Syracuse, New York, area
    • Perform primary care in a post-acute setting for short-term and long-term patients
    • Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
    • Order and interpret lab results
    • Prescribe and monitor medications
    • Consult with physicians and coordinate patient care with other healthcare practitioners
    • Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
    • Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
    • Promote healthy lifestyle changes
    • Requirements: minimum of 1 years' experience as a nurse practitioner in a primary care setting required
    • Post-acute care experience a plus, but not required
    • Active and unrestricted New York NP or PA license required
    • Active and unrestricted DEA with prescriptive authority required
    • Expected compensation is estimated salary range of $115,360 to $122,416 with no cap on productivity income potential
