JOB PURPOSE:

The WCPM shall provide consultation for patients requiring wound, ostomy and continence care. The WCPM sole responsibility hereunder shall be to provide consultation to the organization.

JOB RESPONSIBILITIES:

Primary responsible for the development, maintenance, and oversight of the Wound Care program for the PACE program.

Assist The Learning Center with the oversight of all annual wound competency assessments for all nurses.

Coach on the spot nurses who are not following protocols established in the Wound Care program, as needed.

Provide guidance to administration regarding planning, policy development, and priority-setting related to wound, ostomy & incontinence based on initial and ongoing evaluations of the participants needs.

Provide consultation and patient care in the management of wounds, ostomies, incontinence under established procedures and guideline, across all locations as needed.

Oversee and assist the PACE Community Health Nurses in devising appropriate methods of assessing and documenting skin, wounds, ostomies status, and selecting appropriate wound care and ostomy products.

Develop and implement educational programs for management of wound, ostomy, and incontinence care for agency staff.

Maintains a summary of consultation activities.

Provide wound, ostomy & incontinence consultation to any patient with wound or ostomy in accordance with physician orders and as requested by the sites.

Document, maintain and submit accurate and relevant clinical documentation regarding patients’ condition, findings, and care provided in accordance with the policies of the accepted professional practice.

Serve as point of contact and a resource consultant to all clinical staff for skin, wound, ostomy and continence care problems and any aspects of advanced wound, ostomy & continence techniques.

Make recommendations for appropriate skin, wound and ostomy care and management.

Review and oversee the wound care trackers for each PACE Site

Review and oversee DME supplies ordered by the team to ensure it is appropriate

Field visits for patient care, consultation and teaching, phone consultations as required, and computer use.

QUALIFICATIONS:

Education:

Baccalaureate degree in Nursing preferred.

Licenses (If Apply):

License to practice as a Registered Nurse (RN) in New York State.

WOCN certification.

Other:

Ability to teach and role model for colleagues.

Demonstrated organizational skills, self-directed and independent in fulfilling role.

Highly developed interpersonal skills.

Current New York State driver’s license required.

Access to a reliable vehicle.

Knowledge of electronic medical record.

Physical Requirements

Individuals must be able to sustain specific physical requirements essential to the job. This includes, but is not limited to:

Standing – duration of up to 6 hours a day.

Sitting/Stationary positions – sedentary position for up to 6-8 hours a day for consecutive hours/periods of time.

Lifting/Push/Pull – up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely, using OSHA guidelines, etc.

Bending/Squatting – have to be able to bend or squat to care for patients safely, use medical supplies, etc.

Stairs/Steps/Walking/Climbing – must be able to maneuver stairs safely, climb up/down and walk to access work areas Position requires an individual to be able to travel, walk between sites/locations and work areas throughout the day.

Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (i.e., typing, use of medical supplies, equipment, etc.)

Sight/Visual Requirements – must be able to visually assess patients, read orders and type/write documentation, etc., accurately.

Audio Hearing and Motor Skills (language) Requirements – must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual.

Cognitive Ability – Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.

Salary Range (Min-Max):$110,000.00 - $120,000.00
