P U R P O S E

O F

J O B The Resource Navigator is an active member of the Senior Health and Wellness Team and provides support services to clients with behavioral health needs. Under supervision of the Health and Wellness Lead, the Resource Navigator will function as a role model; exhibiting competency in supporting the needs of older adults and use of coping skills; serve as a consumer advocate, providing consumer information and peer support for clients at Silver Key.

D U T I E S

A N D

R E S P O N S I B I L I T I E S The Resource Navigator performs a wide range of tasks to assist clients in regaining independence within the community and mastery over their own recovery process.

Using a formal goal setting process, the Resource Navigator will provide support and services which may include but are not limited to:

Mentoring and Education – providing education on the use of internal and external services to support

Phone Support - Handle all inbound calls, directing them to relevant departments or providing information and referrals as

Advocacy – modeling and teaching advocacy skills as it relates to getting health needs met.

Recovery and Wellness Support- providing prevention and early intervention, outreach, assessment, and education aimed to support recovery and ongoing

Facilitate Communication – connecting seniors and other applicable entities such as landlords, utility companies, benefit programs, etc

Application Assistance – guiding and supporting navigating available

Financial - budget assessments and emergency assistance to promote long term stability.

Health and Safety – support and guidance with accessing healthcare, safety needs, and services.

Housing – assistance and support with maintaining, locating, or securing affordable housing.

Transportation – support and assistance with acquiring transportation, as

Nutrition Assistance - assessment of nutritional needs and connection to nutritious food sources such as government commodities, emergency food, food stamps, Silver Key Connections Café meals, and Silver Key Meals on Wheels

Veterans Assistance – support navigating veteran specific challenges or barriers

For candidates who meet qualifications, this position may be evaluated for the opportunity to achieve standing as a peer support specialist.

D R I V I N G

D U T I E S Will occasionally be required to drive on behalf of Silver Key Senior Services. Must maintain a current, valid Colorado Driver's License. Must maintain the state issued minimum automotive liability insurance coverage. Must observe and obey all federal, state, and local traffic laws. Responsible for the proper function and maintenance of any Silver Key vehicles used in performing assigned duties

R E Q U I R E D E D U C A T I O N a n d / o r E X P E R I E N C E

High School Diploma or equivalent

This position requires personal experience with Mental Health Services, Behavioral Health Services, and/or recovery from Drug or Alcohol This qualification will give the Resource Navigator the ability to navigate through these services as an experienced mentor for clients to provide holistic healing, to foster growth, and to support and maintain independence for Senior's seeking these wrap-around services.

Knowledge of the Resilience building process and the ability to facilitate recovery using established standardized mental health

Knowledge and skill to teach and engage in basic problem-solving strategies to support individual clients in self-directed recovery.

Knowledge of the signs and symptoms of mental illness (i.e. auditory and visual hallucinations, aggressive talk and behavior, thoughts of self-harm or harm towards others, isolation) and the ability to assist the client to address symptoms using strategies such as positive self-talk.

Knowledge in utilizing community resources necessary for independent living and ability to teach those skills to other individuals with severe mental illness and the Resource Navigator may accompany clients to community resources to assist them in accessing these resources.

Knowledge of how to establish and sustain self-help (mutual support) and educational groups by soliciting input from mental health consumers on their strengths and interests.

Frequent local travel may be necessary. Driving and/or transportation is required for logistical coordination of medical appointments, job sites, outreach events, social activities and ensuring client access to other community resources.

Advanced education and/or experience may be substituted for the education and experience indicated above.

T E C H N O L O G Y

P R O F I C I E N C Y This position requires good proficiency in the use of computer software and applications, office equipment, and professional telephone use. The ability to adequately type quickly with few to no errors is also very important. This position will often require multi-tasking in the use of computer, phone, and office equipment while

interacting with clients. Candidates for this position may be required to take and pass technology proficiency assessments prior to employment.

S K I L L S Demonstrated strong verbal and written communication skills, analytical skills, statistical and mathematical abilities, and interpersonal skills. Ability to read and interpret documents such as safety rules, operating maintenance instructions and procedure manuals. Ability to write complex reports and distill information for many audiences. Proficiency in non-English languages is highly valued.

S K I L L S Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
