Schedule: Full-time, Mon – Fri, 8a – 4:30p

Pay: Salary based on experience plus full benefits

Job Description

The Assessor will conduct person-centered assessments to determine eligibility for various programs and services; provide education and support to individuals and their caregivers and ensuring that all documentation and compliance standards are met; work collaboratively with internal staff and external partners to advocate for clients and connect them with appropriate resources.

Requirements

• LSW or RN license in good standing in Ohio
• At least one year of experience in medical social work, home health care, and/or geriatrics
• Good working knowledge and understanding of issues related to the elderly population desired
• Valid driver’s license with good record and reliable transportation
