Job Description

Social Worker

Full-time / Day-Evening (Mon-Fri 10:30am-7:00pm)

UM Charles Regional - La Plata, MD

General Summary

Under general supervision, coordinates all systems and services required to provide patients and families social support and manages the discharge planning to ensure quality, cost-effective care, and efficient utilization of resources across the healthcare continuum. Patients served include neonates through adult-geriatric, with a primary focus on emergency department patients.

Qualifications

Education and Experience

• Licensure in social work in the State of Maryland.
• Master’s degree in social work; two years of hospital or social agency experience preferred.

Knowledge, Skills, and Abilities

• Knowledge of social/welfare programs including Medicaid, social security, and disability.
• Understanding of regulations governing Maryland nursing homes.
• Ability to provide age-appropriate social counseling.
• Knowledge of growth and development principles across the lifespan and ability to assess patient data accordingly.
• Maintain confidentiality and rights of patients and departmental documents.

Compensation: Pay Range: $31.78-$44.50
