Overview

CommonSpirit Health is seeking a Social Worker for Home Health & Hospice to provide compassionate, personalized care in the home. The Social Worker will assess psychosocial needs, develop care plans, coordinate with the medical team, and support patients and families throughout the care continuum, including discharge planning and bereavement coordination.

Responsibilities

  • Assess the psychosocial status of clients related to illness and environment and communicate findings to the registered nurse.
  • Carry out social evaluations and plan interventions based on evaluation findings.
  • Maintain clinical records for all clients referred to social work.
  • Provide information and referral services to clients and families/caregivers regarding environmental needs and connect with community agencies.
  • Promote agency programs and events in the local community.
  • Serve as liaison between clients/families/caregivers and community agencies.
  • Maintain collaborative relationships with organization personnel to support client care.
  • Develop and maintain contracts with public and private agencies as resources for clients and staff.
  • Participate in the development of the total plan of care and attend case conferences as required.
  • Assist the medical team in understanding significant social and emotional factors related to health problems.
  • Participate in discharge planning and coordinate new volunteer training, continued education, and annual evaluations.
  • Assign volunteers to hospice families and coordinate visits; coordinate the bereavement program.
  • Be the contact with the foundation department at the local hospital in conjunction with the agency director.
  • Ensure proper documentation of social services in clinical records in compliance with state, federal, and Joint Commission standards.
  • Other duties as delegated by the Clinical Director/Supervisor.

Qualifications

  • Bachelor's degree in Social Work
  • Licensed as a Social Worker in Minnesota and North Dakota
  • One year of experience in healthcare
  • Excellent verbal and written communication skills and strong organizational skills
  • Basic computer skills

Preferred

  • Master’s degree in Social Work (salary dependent on experience)
  • Experience in Home Health and Hospice

Pay Range

$27.43 - $40.80 /hour
