Job Description: MSW Social Worker
This worker will operate under the supervision of the Director of Social Work but will have significant autonomy and specialized expertise in psychiatric services. They will collaborate with an interdisciplinary team caring for patients across various units of the Medical Center.
The MSW Social Worker reports to and is accountable to the Director of Social Work. They work with all staff on the units as needed for patient care, quality management, committees, and focus groups, serving in a consultative role to support staff and patient care.
Authority
Provide social work services on assigned units, helping patients and families cope with plans and decisions before, during, and after hospitalization. Supervise and train graduate social work students and other students when appropriate and approved, meeting the requirements for supervision as per the respective Graduate School of Social Work.
Responsibilities/Essential Functions
- Customer Service: Provide excellent service to internal and external clients, customers, and patients, adhering to Service Excellence Standards.
- Patient Support: Offer individual and group counseling, support patient advocacy, conduct diagnostic evaluations and psychosocial assessments, and provide referrals and follow-up services.
- Team Collaboration: Work with clinical interdisciplinary teams, facilitate continuing care plans, and participate in staff in-services.
- Crisis and Case Management: Provide crisis prevention, case management, and resource assistance, including referrals to community agencies and financial aid support.
- Documentation: Maintain timely documentation of social work activities in patient records.
- Consultation and Leadership: Consult with medical center team members regarding psychosocial needs and lead or co-lead support groups for patients and families when appropriate.
- Community Engagement: Participate in community organizations relevant to the patient population.
- Patient Assessments: Conduct psychosocial assessments for high-risk clinic patients, prioritizing problems within limited time, including those with mental illness, abuse histories, or substance abuse issues. Support patients during crises and assist with concrete services, including referrals for adoption when needed.
Minimum Education
A master's degree from an accredited school of social work is required. Licensure as LCSW or LICSW is preferred.
