Gentiva Hospice is hiring: Social Worker MSW in Daytona Beach

Overview

We are looking for a Social Worker (MSW) to join our team. The Social Worker is a member of the interdisciplinary team whose primary function is to provide psychosocial support to the patient/family unit based on an assessment that identifies needs, goals, interventions and services indicated in accordance with the established plan of care, utilizing professional training and personal judgment in monitoring the psychosocial assessment process.

Responsibilities

• Provide psychosocial support to the patient and family unit based on psychosocial assessment findings.
• Identify needs, goals, interventions and services and implement them as indicated by the plan of care.
• Monitor and document the psychosocial assessment process with professional judgment.

Qualifications

• Master’s Degree in Social Work from a CSWE-accredited school.
• Licensure as required by the state in which the hospice operates.
• One year of experience as a Social Worker in a healthcare and/or hospice setting is required; three to five years preferred.
• Knowledge of terminally ill patients and their families; understanding of hospice and the psychosocial dynamics of illness, loss, and death; ability to apply knowledge of the special needs of hospice patients and families; sensitivity to life and death issues; tact in dealing with patients and families; ability to support others at times of crisis; ability to cope with the stress of repeated loss.
