Our values and participant focus lead the way no matter what. The WelbeHealth Social Worker acts as an integral part of the IDT team to help our participants achieve their full potential.

Job Responsibilities

• Conduct social work assessments to determine the psychosocial needs, preferences, and goals of the participants and actively participate in IDT meetings to develop participant care plans.
• Deliver and document social work interventions as outlined in the care plans, including arranging resources, assisting with care transitions, and providing counseling and case management.
• Work with primary care physicians and other care team members to facilitate smooth care transitions between settings.
• Coordinate and facilitate care conference meetings to ensure effective care coordination among team members, participants, and support networks.
• Provide discharge planning when participants disenroll from the program.

Job Requirements

• Master’s Degree in Social Work (MSW) required.
• Minimum of one year experience with a frail or elderly population.
• Experience in coordinating and facilitating care conference meetings.
• Experience assisting individuals with behavioral health and substance abuse issues is preferred.

Benefits and Compensation

Salary range: $74,612 - $98,488 per year plus bonus and equity.
