Under the supervision of the Lead Scheduler, Scheduling Manager or Sr. Director

Operational Support & Innovation, PACE Southeast Michigan (SEMI), the Scheduler is responsible for scheduling Participant Appointments for all visit types: Assessments, Acute Visits, Specialty Appointments, Ancillary Appointments, and Care Plan appointments. Specific Duties And Functions: Answers the telephone and escalates calls to the appropriate party or takes detailed messages for the person and ensures they receive the message Enters participant appointments onto the calendar using the EMR

TruChart or EPIC as appropriate. Verifies that all appointments are authorized prior to scheduling the appointment. Troubleshoots any issues with authorizations or appointments with the RN Case Manager or designee. Ensures transportation requests are completed and input into TruChart following the transportation request workflow. Confirms all appointments with the participant and/or caregiver as appropriate and documents response in TruChart following the appointment confirmation workflow. Ensures that all schedules are reviewed the day prior to the appointment with the RN Case Managers by 1:00 PM to ensure any issues are resolved. Tracks no-shows and cancellations and informs the RN Case Manager of all cancellations; reschedules missed appointments following the workflow for rescheduling appointments. Discharges participants after clinic visits and schedules any needed follow-up. Prepares packets for participants to take to their Specialty Appointments as directed by the Clinic Coordinator or RN Case Manager. Prints and provides a copy of the daily schedule to the Primary Care Team. Present for the Participant Care Team (PCT) meeting 2x per day when needed and follows up on any issues identified by the team. Performs other duties as assigned. Knowledge, Skills And Abilities: Requires a level of knowledge that is normally associated by completion of high school or equivalency. Requires the ability to function in a multi-task environment. Must be self-directed, flexible and committed to the team concept. Requires proficient knowledge of Microsoft Windows programs such as Word and Excel (Data Entry and spread sheets) Working Conditions/Physical Requirements: Prolonged periods of sitting at a desk and working on a computer Must be able to lift-up to 15 pounds at a time Works in a clinical office setting May travel to/cover other sites as needed Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
