This Administrative Coordinator, with direction from the Director, Outpatient Care Management is responsible for triaging referrals to the care management staff from various sources including referrals received through the department phone, alerts or reporting from Epic, and other applications that are used to identify patients for care management services. In addition, the coordinator is responsible for tracking a caseload of patients who transition from inpatient to short-term facilities, and then triaging them to the care management staff once discharged home or a home setting. The coordinator is responsible for keeping care management practice assignments and provider lists up to date to ensure appropriate assigning of patients for care management services. Friday 8a to 4:30p. Work Performed Job Duties and Responsibilities Essential Functions: Triage
assigning patient referrals for care management services to the appropriate care management staff. Short-term facility patient tracking with triage post discharge to the appropriate care management staff. This includes outreach to short-term facilities for patient updates on therapy progress, labs, and discharge planning. Kronos
payroll entry Basic knowledge of NCQA standards. Collects care management staff, other SLUHN departments, and community-based organization requests that provide education on disease management, services, and community resources available to patients. Schedule and coordinate educational presentations during staff meetings. Take minutes for staff meetings and other committee meetings as assigned. Schedule rooms for meetings. Order office supplies, PPE supplies, and minor equipment for care management staff. Maintains all department contact lists and equipment lists. Publishes department Newsletter Mails welcome packets, patient satisfaction surveys, and BPCI-A letters to patients as needed. Works with Director or Supervisors to coordinate orientation schedules. Onboarding access to Athena Forum for new employees. Answers the telephone, makes appointments, refers callers to appropriate individual/department as necessary, runs errands and distributes mail. Organizes and maintains updated I-drive Care Coordination folder information. Maintains confidentiality of all materials and adheres to appropriate protocols for the release of information. Must be pleasant and courteous in dealing with others with ability to positively address challenging situations. Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external). Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes, and practices. Complies with Network and departmental policies regarding attendance and dress code. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated. Other Functions: Works with other staff as assigned. Medical staff social support. Other related duties as assigned. Physical and Sensory Requirements: Sitting for up to 7 hours per day, 3 hours at a time. Frequently uses fingers for typing, data entry, etc. Frequent use of hands. Uses upper extremities to occasionally lift up to 35 pounds. Occasionally stoops, bends, or reaches above shoulder level. Hearing as it relates to normal conversation. Seeing as it relates to general vision, near vision, peripheral vision, and visual monotony. Potential On-the-Job Risks: No identified risks. Most Complex Duty: Setting priorities for accomplishing tasks in a timely manner. Accurate tracking and communication of CME approvals and completion. Supervision (Received and/or Given): Receives supervision from Director, Outpatient Care Management. Outpatient Care Management, Supervisors instruct and offer guidance in care management matters relating to their respective area. Communications: Communicates with care management staff, leadership, short-term facilities Additional Requirements: Knowledge of medical terminology, general office protocol and ability to take concise accurate meeting minutes. Must be able to deal with confidentiality on a daily basis. Minimum Qualifications: Education: Associate degree with 3 years secretarial experience. Bachelor's degree with 2 years secretarial experience. Training and Experience: Proficient in Word, Excel, and PowerPoint preferred. Work Schedule: Day shift with varying hours based on the needs of the department.
