Overview

In this role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Responsibilities
- Support and advocate for patients by guiding them in managing rights, responsibilities and the demands of End Stage Renal Disease.
- Collaborate with the care team to coordinate services and ensure patient-centered care.
Qualifications
- Meets all state required regulations to practice in the dialysis setting.
- Master's degree in Social Work (MSW) required with a specialization in clinical practice.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook and familiarity with DaVita applications within 60 days.
