JOB SUMMARY
The Social Worker, Bachelors of Social Work (BSW) is part of the total healthcare team to help meet the straightforward social, economic and emotional needs of patients and families. The individual will conduct bio/psycho/social assessments with patients to aid in identifying the needs, wishes, motivation and strengths of the patient as a participant in their healthcare. The Social Worker, Bachelors of Social Work (BSW) will coordinate referrals and patient care needs for health services in the home and community. This individual will primarily care for child/adult/a combination of child and adult populations when performing the functions of this role.

JOB QUALIFICATIONS
- Education
- Required Education: Bachelor’s degree in social work.
- Preferred Education: Master’s degree in social work
- Experience
- Minimum Experience Required: Three years social work experience. Demonstrated proficiency with interpersonal and communication skills.
- Preferred/Optional Experience: Experience working as a Social Worker in a healthcare facility or community health setting working with children and adults.
- Certifications/Licenses
- Minimum Required: Certified Social Worker (CSW) license awarded by applicable state.
