Overview
Be part of an innovative program in Massachusetts. CCBC (Community Counseling of Bristol County) and the Department of Mental Health (DMH) collaborate on the PACT-Y program to support youth and families with a creative, flexible, dynamic team. The Family Support Partner role focuses on establishing and maintaining engagement with youth and families, leveraging lived experience to provide guidance and mentorship. Responsibilities
Work as part of a multi-disciplinary team to provide the caregiver perspective and ensure treatment goals support family-collaboration principles Teach advocacy skills and provide education on identifying natural supports, advocating for youth within educational systems, and obtaining resources to improve family functioning Share experiences of raising a child with emotional and behavioral health needs Work in the home and community to provide coaching and support to caregivers Coordinate care with the family, team, providers, and DMH Complete required documentation per program guidelines Requirements
Submit a resume and cover letter describing the lived personal experience requirement Experience as a caregiver of youth with mental health and/or co-occurring substance use challenges High school diploma or GED with a minimum of 2 years working with children/families, or an Associate degree in a human service field with 1 year of experience, or a Bachelor’s degree in the human services field with 1 year of experience Must have a current, valid driver’s license and reliable transportation Job details:
