Coordinate support services such as transportation, housing assistance, or financial aid for patients and families navigating the healthcare system.

Facilitate communication between healthcare providers, patients, and families to ensure understanding of care plans and available resources.

Maintain documentation and case records to support continuity of care and compliance with institutional policies.

Advocate for patient rights and access by working with insurance providers, legal aid, and community organizations.

Develop and manage resource directories and referral networks to connect patients with external support programs and social services.

Additional Skills & Qualifications
• Requires a minimum of a Master’s degree in Social Work, Clinical Psychology, or Psychology. A concentration in Gerontology, Pediatric, Public Health, Substance Abuse, Mental Health or other related fields is preferred
• Requires a minimum of five years of clinical work experience in the above field
An active, unrestricted Licensed Clinical Social Worker license substitutes for two years of work experience

Pay and Benefits
The pay range for this position is $39.50 - $39.50/hr.
