Overview
The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include but are not limited to administration of medications, documentation of medication administration, conducting SHUs, and providing resident care while demonstrating the Sunrise Senior Living mission to champion the quality of life for all seniors in accordance with federal, state/provincial, and local laws, standards and regulations and Sunrise Senior Living policies.
Responsibilities
- Provides the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
- Reviews, reads, notates, and initializes Daily Log to document information about residents.
- Receives medication updates from the Resident Care Director (RCD) or Wellness Nurse.
- Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass.
- Ensures medications are passed according to times and documents as medications are given, including refusals or missed doses.
- Maintains confidentiality of all resident information including medication details.
- Reports resident concerns to the RCD or Wellness Nurse.
- Restocks medication cart after passes and assists in checking medications regardless of packaging system.
- Assesses residents to determine the need for "as needed" medication and documents/reports to the supervisor.
- Counts narcotics with another Medication Care Manager or Lead Care Manager each shift.
- Maintains and cleans the Medication Room, medication carts, and treatment carts; follows refill processes.
- Promotes resident independence, dignity, and safety in line with Sunrise Principles of Service.
- Interacts empathetically, acknowledging each resident’s communication patterns, history, and needs.
- Adheres to standard infection control practices and safety precautions.
Resident Care
- Notifies RCD of resident or family concerns through the LEAD process.
- Participates in daily Cross Over meetings and conducts Service and Health Updates as directed by RCD.
- Contributes to the development of ISP/ICP plans and monthly updates; transcribes orders when required.
Risk Management and General Safety
- Ensures compliance with OSHA/Workers Compensation Act and Occupational Health and Safety regulations; promotes risk management programs.
- Practices safety procedures including PPE, fire safety, SDS, and lockout/tagout where applicable.
- Reports accidents, unsafe conditions, and hazardous equipment promptly.
- Keeps cords, carts, equipment clear of exits; stores oxygen tanks safely and monitors prescribed levels.
Training and Team Success
- Actively participates as a team member and supports team goals.
- Demonstrates Sunrise Team Member Credo and Principles of Service in daily interactions.
- Engages in team and resident engagement programs and attends required meetings and trainings.
- May serve as shift supervisor or supervise other Medication Care Managers; maintains compliance with required training and regulations.
Qualifications
- LPN, LVN, or state/province licensed nurse credential; appropriate certifications as required by state/provincial regulations.
- CPR and First Aid certifications.
- Must be at least 18 years old; prior experience with seniors preferred.
- Ability to make decisions in residents’ best interests; remain calm in difficult situations; handle multiple priorities; communicate effectively; exercise good judgment.
- If applicable, drivers of Sunrise vehicles must sign the Driver job description and comply with safety duties.
