POSITION SUMMARY – Social Worker

Responsibilities

• Primary role is an inpatient social worker at a 25 bed critical access hospital, with involvement in outpatient areas including ED, Surgery and provider practices as needed.
• Assess the psychosocial functioning of patients and families, and intervene to connect patients and families to necessary resources and supports in the community; provide supportive counseling or grief counseling as needed.
• Ensure appropriate access to care and resources and an environment that is safe for discharge.
• Conduct psychosocial assessments, crisis intervention, and linkage with community and financial resources for inpatients, outpatients, families and significant others as appropriate.
• Provide care to all age groups, with the majority of care delivered to the adult/geriatric population.
• Make independent decisions within scope of practice and in collaboration with team members to assist patients and families with complex social situations related to hospitalization, illness, diagnosis, treatment, and life circumstances that may include emotional, mental, and substance abuse considerations.

Qualifications

• LCSW
• BLS
• 3 years social work experience in an acute hospital setting

Contract Details

• LENGTH OF ASSIGNMENT – 13 weeks
• SHIFT / HOURS PER WEEK – 8am - 4:30pm (40 hrs/wk)
• SYSTEMS – Epic
• START DATE – ASAP
