Harbour View Senior Living, an Assisting Living And Nursing home in North Bergen NJ, is looking for a Full Time license social worker with experience to join our dedicated team of healthcare professionals.

Responsibilities

• Develop preliminary / comprehensive assessments for each resident.
• Encourage the resident / family to participate in the development and review of resident’s plan of care.
• Discharge planning
• Develop a written plan of care for each resident that identifies the social problems as well as the needs of the resident and set goals to be accomplished for each problem / need identified.
• Ensure that the social services components of the MDS assessment are completed and signed in an efficient manner.
• Review nurse’s notes to determine if the care plan in place is being followed.
• Ensure quality customer service.
