Responsibilities
- Performs and documents highly complex social services work, serving as a liaison between the patient, legally authorized representative, and others to assure ongoing care, treatment, and support using person-centered practices.
- Gathers information to assess the individual’s support systems and service needs; participates in interdisciplinary team meetings as needed; may assess individual rights and capacity to make decisions.
- Designs and provides individual and group trainings to promote exercise of rights and patient transition, including curriculum, materials, and case-specific information.
- Assists with coordination of furloughs, transfers, transitions, and discharges as needed; focuses on complex cases and post-discharge placement and support.
- Works under limited supervision with substantial latitude for initiative and independent judgment.
- Performs other duties as assigned, including participation in disaster response/COOP activation and potential alternate shift patterns.
- Assists with supervision of Social Worker I/II and social work students, including interviewing, hiring, training, evaluations, and counseling as needed.
- Oversees transfers, transitions, furloughs, and community placement discharges, coordinating with patients, families, courts, LMHA, and treatment providers to ensure orderly discharge and continued treatment.
- Designs and conducts individual and group trainings to promote rights, decision-making, and self-advocacy; develops curriculum and training materials.
- Participates in discharge program meetings and internal/external work groups; identifies service gaps and leverages existing resources.
Knowledge, Skills And Abilities (KSAs)
- Knowledge of human growth and development, mental health, and social work standards; knowledge of facility and community resources.
- Knowledge of discharge policies and procedures for state hospital patients with high/complex needs.
- Ability to adhere to Code of Conduct and Ethics, and to work collaboratively with others on behalf of patients.
- Effective written and verbal communication; skills in interviewing, assessment, planning, intervention, evaluation, case management, mediation, counseling, information and referral, problem solving, supervision, and advocacy.
- Ability to supervise others and accountability for assigned property with limited financial responsibilities.
Qualifications
- The Social Worker is required to maintain licensure in accordance with the Texas Board of Social Work Examiners.
- Initial Screening Criteria:
- Minimum Licensed Bachelor’s degree in Social Work (LBSW) and two years of social work experience with persons with mental, social, or substance use disorders, OR
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with one year of relevant experience; or completion of State Hospital internship program.
Additional Information
- Applicants must pass fingerprint background, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, Employee Misconduct, LEIE).
- All applicants must be at least 18 years old; Waco Center for Youth applicants must be 21.
- Flexibility in work hours; overtime or extended hours may be required.
