SALT Outreach Inc. operates a mobile drop-in center for people experiencing homelessness. Currently, SALT operates five days a week in downtown Orlando, serving up to 200 people per day with 12 different services which includes case management and mental health counseling. Our mission is to share Christ's love by serving the unsheltered through comprehensive mobile drop-in centers. Our vision is that every metropolitan area in the US has a comprehensive drop-in center for people experiencing homelessness.SALT has seen rapid growth over the past few years and has a team of 47 people. We are looking to expand our team with people who have hearts for our friends experiencing homelessness. Description
Some travel would be required (Monday to Friday) in the surrounding areas of our SALT locations in Bithlo & Apopka. The Field/Street Outreach Case Manager will explore camps, wooded areas, parks, abandoned buildings, or other places in Orange County where the unsheltered may frequent in hopes of connecting them to services. Effective street outreach reaches people who might not otherwise seek assistance or come to the attention of the homelessness service system. Responsibilities and Duties: The functions of this position include: a. Will explore unsheltered locations such as parks, encampments, cars, abandoned buildings, wooded areas, and identify unsheltered clients. b. Will complete engagement forms with unsheltered clients to determine needs and inform them about SALT services and other services in the community that are available to them. c. Possibly provide transportation/transportation resources for clients to participate in programs and services. 2) Referrals a. Will refer clients to organizations to obtain services and resources not offered through SALT. b. Will hold clients accountable to referral and follow up with client, system, or organization to confirm referral was followed through. c. Will keep and monitor a log of each organization in Central Florida that supports the unsheltered. Updating the log as needed 3) Case Management a. Identify candidates eligible for Case Management Services b. Assist each client with getting connected to the appropriate case manager c. Empower them to pursue their goals regarding obtaining housing (or employment) and assist in holding them accountable to those goals. 4) Documentation a. Will keep detailed internal notes for each client in a secure location. Will ensure privacy and confidentiality of all notes taken. b. I will obtain signed authorizations to obtain and disclose information before entering info in HMIS. c. Documentation will be done through the Homeless Management Information System (HMIS) and SALTs internal note system to support the Continuum of Care and must be completed within one day of sitting down with a client. d. Monthly reports may need to be produced and presented to the management team. 5)
Supervision a. May assist in orienting new social work interns. b. May assist in conducting training with social work interns as a group and individually. c. Be an ongoing resource and coach for interns throughout their internship experience. 6)
Transportation a. Will drive to designated areas for street outreach at the direction of senior case managers. b. Will use company vehicles for transportation to services when applicable. c. May use company vehicles to purchase supplies or services for clients. Qualifications: Requirements: Must have a bachelors in social work or related field Excellent verbal and written communication skills Strong problem-solving and interpersonal skills Organized accurate, thorough, detail-oriented, and able to monitor work for quality Must be able to stand and walk for prolonged periods of time May need to walk/stand for up to 8 hours at a time with a standard break Intermediate level with MS Excel, MS Word, and Google Docs. Must have a valid drivers license and personal vehicle. Preferences Experience in working with the homeless. Experience doing administrative work.
