We are seeking a full-time Discharge Planner to join the Case Management team at Phoenix. The Discharge Planner is responsible for facilitating care along the continuum through effective resource coordination, helping patients achieve optimal health, access to care, and appropriate use of resources—all balanced with the patient's resources and right to self-determination. Duties and Qualifications:

This position provides clerical support to Case Managers and Social Workers. A B.S. in Sociology or Psychology is required, with a minimum of one year of experience in a behavioral health environment. BLS (American Heart Association) certification for healthcare providers is required upon hire, and CPI certification must be obtained within 30 days of hire. Major Tasks, Duties, and Responsibilities: Collaborates with physicians, therapists, the treatment team, and patients to develop effective continuing care plans. Makes telephone calls, handles faxing and copying, and coordinates discharge plans. Provides patients with reasonable aftercare options and discusses these options with the patient individually or with family, as requested. Collects and releases information to accurately research and refer to services. Maintains timely and accurate records. Initiates contact with referral sources, community mental health providers, physicians, nursing homes, residential treatment facilities, halfway houses, and other placement options to facilitate continuity of care. Coordinates admissions to secondary hospitals, treatment centers, nursing homes, halfway houses, and other placements as needed. Schedules follow-up appointments with psychiatrists and therapists for continued outpatient treatment whenever possible. Attends and contributes to treatment team meetings in a punctual and professional manner. Maintains positive relationships with community resources and referrals, representing the facility professionally. Actively researches and updates provider, treatment facility, and halfway house lists. Maintains an up-to-date referral, contact, and resource database. In addition to base pay, additional compensation may be available for CMC Phoenix roles, including shift differential and other special pay practices. The posted compensation is a reasonable estimate that extends from the lowest to the highest pay that CMC Phoenix in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
